What is Case Management?
Case聽Management聽is a patient management program designed to improve the health of patients with chronic illnesses such as Diabetes, High Blood Pressure, Heart Disease, Asthma, COPD and High Cholesterol. Initially a聽head-to-toe聽physical and an in-depth patient chart review are performed, followed by聽the聽development of an individual treatment plan. The Case Management staff will explain our preventive health procedures that include services such as聽vaccines.
